What to Bring With You to Urgent Care (ID, Insurance, etc.)

Whether it’s your first time visiting urgent care or you’re a returning patient, it helps to know exactly what to bring with you. Being prepared can help reduce wait times, avoid confusion, and make your visit smoother from start to finish.

At North Smithfield Urgent Care, we accept walk-in patients of all ages — no appointment required. Here’s a quick guide to help you arrive ready.

1. A Valid Photo ID

A government-issued photo ID is required for all patients 18 and older. This helps verify your identity and ensures your medical records and billing information are accurate.

Common forms of accepted ID include:

  • Driver’s license
  • State ID
  • Passport
  • Military ID

For minors, a parent or guardian must accompany the child and provide identification.

2. Insurance Card (If You Have One)

We accept most major insurance plans, and having your insurance card helps us bill your provider correctly. Please bring the physical card or a digital copy that includes:

  • Policy number
  • Insurance provider name
  • Group number (if applicable)
  • Subscriber name

3. Payment Method (If Self-Pay or Copay Applies)
